If it's your first appointment we also need your forms ( Health History Questionaire and sample 3 day food diary ) completed and emailed to clinic at least 48 hours before your appointment. (We have other formats of this document please ask if you need an alternative.)

 If appropriate Yvonne may recommend tests or supplements to support your programme, these would be at additional cost to the consultation. These should be paid direct to supplier or laboratories not 'Foods for life'

If you need to change your appointment please give a minimum of 48 hours notice. If this notice is not given a £35 cancellation fee will be charged. If you cancel on the day you will be charged the full cost of the consultation.

 

Where to go

Your consultation will be at the Oasis Health Centre, 98 High Street, Croydon, CR0 1ND in the therapy rooms above the Buddhist Centre. There is a car park at the rear in Wandle Road. There is also a bus stop directly outside the centre which is on many bus routes. George Street or Reeves Corner are the nearest tram stops and East or West Croydon main line are both within a 10-minute walk. There is a map on our website www.optimumnutritionists.com

 

When you arrive please go into the Buddhist Centre shop, they will direct you to an upstairs waiting area. Alternatively or if the shop is closed please press the buzzer by the door between the charity shop and Buddhist shop. The health centre buzzer is the top button. Your therapist will then let you in.
