To confirm your Harley St clinic appointment

If it's your first appointment we also need your forms ( Health History Questionaire and sample 3 day food diary ) completed and emailed to clinic at least 48 hours before your appointment (We have the forms in other formats Please ask if you require an alternative format)

If your payment is not covered by selections above please use this button below and enter in your own amount - but please enter what the payment is for and if appropriate the appointment date and time. (Please note supplements and tests should be paid for direct to suppliers / laboratories not Foods for Life Health & Nutrition. Cheques should be made payable to 'Foods for Life' Contact us for bank details should you prefer to make a direct transfer. If you pay for your appointment in cash please advise clinic support team and ask the nutritionist for a reciept.
If appropriate Yvonne may recommend tests or supplements to support your programme, these would be at additional cost to the consultation and paid directly to suppliers or relevant laboratories.
If you need to change your appointment please give a minimum of 48 hours notice otherwise a cancellation fee of £35 will be charged. If you give less then 24hrs notice, cancel on the day or do not turn up you will be charged the full cost of the consultation.
You can register for a prepaid cheaper standby appointment if your availability is flexible and we will text you to let you know 24 - 6 hrs before your optional appointment (due to last minute cancellations 1pm - 8pm )
Where to go
The Foods for Life Harley Street Nutrition Therapy Clinic is held at 1 Harley Street, Cavendish Square, London, W1G 9QD . Located to the North of Oxford Street in the North East corner of Cavendish Square, north along from the back entrance of John Lewis.
Nearest tubes Oxford Circus and Bond Street underground stations. Nearest buses include Margaret Street 22, 55, 88, 453, C2 . Once you arrive speak to the receptionist and she will guide you to Yvonne’s clinic.
If your Harley Street appointment is after 5.30pm then reception will be closed. Please then go to number 7 Harley Street (next door along). On the door entrance keypad press 21 and then the ‘call’ button. Yvonne will answer the call and buzz the door open. You will then need to up the stairs to the second floor door where Yvonne will meet you. Please do not just go through straight through the street door if it has been left open as Yvonne will not then know you are there and you may get stuck between security doors.
